Lab Policies
Our Commitment:
We are overly committed to your satisfaction and our consistency, quality and reliability. We will guarantee every appliance that leaves our laboratory to fit the model provided to us. We suggest that the doctor check all models and impressions for distortion prior to sending them to the lab.

We are also committed to excellent customer service. We are only a phone call away if you ever have any questions, comments or suggestions. Since we are a small, family owned and operated lab this helps us to maintain our high standards that we expect from ourselves and that you, our customer has become accustomed to.

Delivery Time
The normal scheduling of appliances are 3 business days in the lab and study models require 7 business days. Business days/hours are Monday -Thursday, 8:00am - 5:00pm and Friday 8:00am to NOON.  It is important to put an actual return date (1 day before actual appointment) on the prescription form to insure the appliance will be delivered on time.

We pick up and deliver in the Tulsa area on Monday - Friday Mornings. We ship all out of town appliances via USPS Priority Mail w/delivery confirmation for the quickest & least expensive method. We will provide pre-paid shipping labels to our laboratory for your convenience.

How To Send A Case
The State of Oklahoma requires that a prescription form accompany all cases. We can
supply our forms at no charge or you may use your state provided ones. Please label each impression/model and if shipping to us, please wrap models in bubble wrap to prevent breakage. Please wrap study model impressions in a wet paper towel and seal in a zip lock bag. This will prevent them from drying out and becoming distorted.

Removable Appliances
Well defined plaster/labstone models (no die-stone, please) and alginate impressions are acceptable for the fabrication of appliances. We require Maxillary and Mandibular models with a bite registration for cases that require habit breakers, occlusal coverage, pontics or when occlusal interference may be a concern.

Fixed Appliances 
Fitted bands can be poured in the model at your office or sent in a separate bag to be placed by us at no extra charge. We also provide an indirect banding service for an extra charge. For our pedo-partials and habit breakers we require a model of the opposing arch to insure the quality and function of the appliance.

Study Models 
Alginate impressions and plaster/stone models are acceptable for fabrication of study models. Cases where plaster/stone models are intended to be used for the fabrication of study models will be charged a duplication fee.

Billing & Payment
Invoices are sent with every case returned to your office. We recommend that each
invoice be kept to be verified against your monthly statement.  Payment terms - Net 10 from STATEMENT date.  We accept Visa, Mastercard, Company Checks and of course, cash!  All overdue accounts will be charged a late fee.

Warranty, Repairs & Remakes
If the broken appliance is a result of a manufacturer's defect, we will be more than happy to repair/remake it at no charge for up to 6 months with receipt of original models.  Any misuse, abuse and/or inaccurate impressions are not covered under this warranty.   If it is a result of misuse, abuse or inaccurate impressions, we can contact your office and give you an estimate of the repair/remake charge before fabrication if requested.
